Job Description for Licensed Practical Nurse (LPN)

We are seeking a dedicated and compassionate Licensed Practical Nurses (LPN) to provide high-quality, patient-centered care in a Long-Term Acute Care (LTAC) facility.

Key Responsibilities

• Provide direct nursing care, including wound care, respiratory support, and infection control management.
• Administer medications and treatments as prescribed by physicians.
• Monitor and document patient conditions, reporting any changes to the RN or physician.
• Assist with activities of daily living (ADLs) such as bathing, feeding, and mobility support.
• Maintain accurate and up-to-date patient records.
• Collaborate with interdisciplinary teams to develop and implement individualized care plans.
• Provide emotional support and education to patients and their families.
• Ensure compliance with all healthcare regulations, infection control policies, and facility protocols.

Requirements

• LPN license in state of assignment or compact license.
• At least 1 year of Medical Surgical or LTAC experience.
• BLS certification and IV certification or equivalent.
• Immunizations, including TB, Covid, and Flu.
