- Responsibilities :
- Deliver healthcare services and treatment to elderly participants, ensuring their physical, mental, and social needs are met.
- Collaborate with a multidisciplinary team, including the Medical Director, Primary Care Physician, and PACE Director, to provide comprehensive care.
- Use your knowledge and expertise to assess, treat, and improve the health outcomes of frail elderly individuals.
- Qualifications :
- Bilingual (English / Spanish) to communicate effectively with our diverse patient population.
- Minimum one (1) year of recent, successful experience working with frail or elderly populations in a community-based or long-term care setting.
- Must be a license Nurse Practitioner in the State of Massachusetts
- Strong understanding of the physical, mental, and social needs of the elderly.
