PT Case Manager @ CH Home Manitowoc - Home Care PT

JOB DESCRIPTION

CANDIDATE MUST HAVE :

• 3 years PT experience
• active PT license (WI)
• active BLS (AHA)
• valid Driver's License (DMV)

Patients typically have multiple comorbidities and can include (but not limited to) : CHF, COPD, pneumonia, cancer, dementia, kidney disease, hypertension, fall, fracture, infections, cellulitis, post-surgical, wound care, ostomy care, labs, IV infusion, etc.

Support Team : Manager / Supervisor, Lead, Scheduler

Technology / Equipment : EPIC / Computer charting, Laptop, Sapphire IV pumps

Any scrub color acceptable (must be professional).

Traveler must provide own transportation to / from all patient visits. Reimbursement available (restrictions apply).
