This full-time opportunity offers the chance to work in a dynamic medical practice, with resources and training to ensure your success as an Advanced Practice Nurse specializing in geriatric nursing .
Key Responsibilities
Provide high-quality, compassionate care for patients in long-term care settings , focusing on geriatric nursing
Collaborate with nursing staff, medical directors, and specialists to create comprehensive care plans tailored to geriatric patients.
Accurately document visits in an electronic medical record (EMR)
Support acute and chronic care management, ensuring patient needs are met efficiently and effectively as an Advanced Practice Nurse .
Qualifications
Required Qualifications :
Active Virginia Medical License and DEA License
Minimum one year of clinical nursing experience
Strong communication skills, self-motivated, adaptable, and a team player
Preferred Qualifications :
Nursing experience in ICU, Med / Surg, Emergency Medicine , or Hospitalist roles
Experience with care coordination and working in long-term care settings, especially geriatric nursing
