We are looking for Hospice RN Care Manager
Location:- Barre, VT 05641
Number of weeks:- 20
Shift:- Monday-Friday
Required:- VT state license and BLS through AHA
Experience:-
- Experience with hospice case management required
- Experience providing hospice care in a home setting required
- Experience with McKesson/Netsmart EMR preferred
Job Type: Contract
Pay: $75.35 - $80.80 per hour
Expected hours: 36 per week
