Overview:
We are seeking an experienced Registered Nurse (RN) to join our Home Health team, providing exceptional care to patients in need of home health, hospice, and palliative care. This position is responsible for administering skilled nursing care using the nursing process to provide individualized care to each patient.
Key Responsibilities:
- Deliver skilled nursing care to home health patients requiring intermittent professional nursing services.
- Perform the initial evaluation visit for home health patients and regularly reassess their nursing needs.
- Create and update the Plan of Care for each patient and make necessary revisions.
- Provide preventive and rehabilitative nursing care to home health patients.
- Participate in in-service programs to enhance skills and knowledge.
- Supervise and mentor other nursing personnel in a home health setting.
- Ensure high-quality care in hospice and palliative care settings to ensure comfort and dignity for patients nearing end-of-life.
Qualifications:
- Registered Nurse (RN) with current active license state or compact.
Additional Benefits:
- Up to $600 per month lodging reimbursement upon successful completion of the contract.
