Key Responsibilities of the Start of Care Nurse - RN
- Initiate and manage comprehensive patient assessments during the weekend.
- Develop individualized care plans to meet the unique needs of each patient.
- Ensure seamless communication and collaboration with the healthcare team.
- Maintain accurate and timely documentation of patient information and care activities.
- Educate patients and their families regarding the care plan and self-management of their condition.
- Adhere to established health regulations and organizational policies.
Schedule Details
This role involves weekend shifts with every Saturday & Sunday two weekdays for four 10-hour days; prefers Friday – Monday or Saturday – Tuesday
Coverage area : Highland Park, North Chicago, Antioch, Wauconda, Vernon Hills, Grayslake, Gurnee & surrounding.
