Job Description

 We are seeking a compassionate and dedicated Community Health Worker specializing in Intimate Partner Violence (IPV). The chosen candidate will help individuals who have experienced IPV, applying culturally diverse, trauma-informed, empowerment-based advocacy. This role includes case management, care coordination, and outreach services.

 Responsibilities:

 ·         Provide trauma-informed, empowerment-based advocacy to persons within the community who have experienced IPV.

·         Assesses clients' needs, develop care plans, and follow-up to ensure adequate service, providing referrals, where required.

·         Coordinate with healthcare providers, legal resources, and social services to address client needs comprehensively.

·         Conduct outreach & educational activities in Spanish and English within the community to raise awareness about IPV and available resources to IPV victims.

·         Conducts community canvassing, table events, and other outreach efforts to and in collaboration with social service agencies further networking with schools, faith-based organizations, and mental health providers, for victim support.

·         Coordinates in-person and virtual IPV educational sessions and presentations.

·         Participates in related support groups, coordinating and facilitating events.

·         Performs related work as required.

The hours of work are 30 per week. Required to travel to site locations (as needed).

This is a grant funded position. The position is contingent on grant funding.

 

Knowledge, Skills, and Abilities

·   Knowledge of Community Health practices, including facilitation of presentations and data collection

·   Demonstrated knowledge of IPV and the effects it has on individuals and communities.

·   Demonstrated problem-solving skills and a trauma-informed approach to concerns and challenges with clients.

·   Knowledge of technical platforms (Microsoft Suite: Excel, Teams, etc.)

·   Ability to communicate effectively, clearly, and concisely both orally and in writing.

·   Ability to display excellent care services to behaviorally challenged clients.

·   Ability to work flexible work hours including pre-scheduled nights and weekends.

·   Strong organizational skills

 

Qualifications

Associate degree in Human services or related field and two (2) years of experience providing education, direct care services or closely related field to a diverse population.

Bilingual – Spanish required.

Valid Driver’s License required.

 

Preferred Qualifications:

·         Advanced degree in Social Work, Public Health, or related field

·         Deep understanding of health and wellness objectives
