The MPI RN Educator PCF assesses, plans, implements, and evaluates the patient centered diabetes education program at the Diabetes Center. Plans and conducts continuing education programs, participates in the development of education curricula and materials. Participates in case management, and patient care planning and goal setting. Provide diabetes education services and general diabetes self-management education.
Qualifications:
Education, Knowledge, Skills and Abilities Required:
- BSN.
- 1 or more years of experience in diabetes care and education/counseling of patients with chronic disease
- 3 or more years of experience as a nurse, preferably one year in community health or ambulatory care setting.
- Excellent written and verbal communication skills.
- Proficient computer skills that include but are not limited to Microsoft Office and/or Google Suite platforms.
Education, Knowledge, Skills and Abilities Preferred:
- Master's Degree.
Licenses and Certifications Required:
- Valid NJ Registered Nurse licensure.
- Certified Diabetes Educator (CDE) or Board Certified.
- Advanced Diabetes Management (BC-ADM).
