What are the responsibilities and job description for the Manager, Integrated Health Management Services position at HMSA?
Select, train, and manage designated staff, fostering a work environment that encourages teamwork, promotes personal growth and learning, couples empowerment with accountability, learns from failure and celebrates success.
Manage all case management / health coordination activities on their designated island(s) ensuring all assessments, health action plans / care plans are completed and implemented according to Med-QUEST, Medicare, or commercial line of business requirements.
Administer an effective program through multi-disciplinary teams with the goal of improving health care outcomes and member satisfaction.
Identify opportunities and develop programs to improve the operational efficiency and cost-effectiveness of case management / health coordination processes and programs.
Oversee the performance of their team on their assigned island to ensure that appropriate services are being provided. The manager will maintain a positive working relationship and ensure that deliverables are being met in accordance with contract requirements.
Establish and maintain cooperative and productive professional relationships with health coordinators and case managers at various facilities and community organizations.
Act as a liaison and mediator between participants, families, and service providers addressing any concerns that may arise.
Attend and participate in meetings / workgroups / committees for community organizations and serve as resource / liaison between HMSA and the community.
Performs all other miscellaneous responsibilities and duties as assigned or directed.
