Summary:

This job description is intended to clarify the responsibilities and duties of the ROOT Coordinator. The Coordinator will work with a team to respond to individuals who have experienced an overdose event within 24-72 hours. This position is predominantly a community outreach-based program. This position has the primary responsibility for coordinating care between all organizations participating in the ROOT program and community resources. The coordinator will also act as a care navigator for clients who have experienced an overdose event by providing any case management needs throughout the recovery process. It is crucial that the Coordinator is respectful of all pathways of recovery. Administrative duties include writing reports and service delivery problem solving. While this job description lists duties and responsibilities, it cannot be considered as inclusive of all the tasks required.

Responsibilities and Duties:

· Coordinate care with the ROOT team members to best assist client in the recovery process

· Provide recovery related support to those seeking recovery including but not limited to harm reduction, prevention, recovery planning activities, and coordinating service delivery for successful outcomes

· Demonstrate cultural awareness, competency and humility

· Participates in the development and maintenance of the strengths-based assessments for clients, including status, needs, and recovery capital

· Manages client crisis either directly or indirectly by accessing needed services

· Directly assists clients in securing and maintaining needed services and entitlements

· Assist in systems advocacy to secure needed services for the client

· Coordinate with outside agencies to provide holistic care for clients

· Help to build a robust community resource guide for specific population

· Collect, analyze and report data

· Actively participate in all scheduled staff meetings

· Complete all required documentation

· Adhere to all established ethics in accordance with company policy and certification requirements

· Be a recovery advocate for self-determination of recovery goals directed by the client’s needs and wants

Requirements:

· Bachelor’s degree in the human service field or equivalent in another field

· Preferred candidate will also meet the minimum requirements for the MDHHS Certified Peer Recovery Coach (CPRC) credential, which can be found here: https://www.michigan.gov/mdhhs/keep-mi-healthy/mentalhealth/drugcontrol/peer-recovery-information

OR: A person in long-term recovery, family member, ally, or a working knowledge of the resources available to the recovery community

· Have the ability to work independently and in a team environment

· Strong leadership skills and the ability to easily network with other organizations

· Empathetic and supportive to individuals in a crisis situation

· Be enthusiastic about recovery and working with individuals who are seeking recovery

· Self-motivated and take initiative on tasks that need to be completed

· Demonstrate conduct that reflects the values and goals of the agency to be inclusive, non-discriminatory, and free of judgement in interactions with others

· Proficient computer skills (Word, Excel, Publisher)

· Very proficient communication skills; both verbal and written

· Ability to think outside of the box and be a problem-solver

· Current driver’s license and reliable transportation. Employees must provide a copy of the current driver’s license and proof of auto insurance.

Job Type: Full-time

Pay: From $20.00 per hour

Schedule:

  • Monday to Friday, some afternoons, and weekends
