What are the responsibilities and job description for the Targeted Case Manager position at Holistic Wellness?
We are currently seeking a dedicated and compassionate Targeted Case Manager (TCM) to serve individuals in Meade County, KY. This is a flexible opportunity ideal for self-motivated professionals who are passionate about supporting mental health clients and connecting them with the resources they need. Our Ideal TCM will work primarily with children and adolescents as well as their families.
Key Responsibilities
- Provide community-based case management services to a caseload of up to 25 clients
- Conduct assessments, develop service plans, and coordinate care
- Assist clients and their families in accessing services such as housing, medical care, employment, and other resources
- Maintain timely and accurate documentation using our EHR system
- Collaborate with service providers, families, and community agencies
- Attend required training and maintain compliance with program standards

Qualifications
- Bachelor’s degree in Social Work, Psychology, or related human services field (required)
- Certification in Targeted Case Management or willingness to obtain (preferred)
- Strong organizational, interpersonal, and communication skills
- Reliable transportation and valid driver’s license
- Commitment to client-centered, strength-based care
