Are you a compassionate Licensed Master Social Worker (LMSW) or Hospice Chaplain with a deep passion for hospice care and leadership? Faith Hospice seeks a dedicated Manager of Psychosocial and Spiritual Care to lead our team of Hospice Social Workers and Spiritual Caregivers in providing exceptional support to patients and families at the end of life.
What You’ll Do
Since 1995, Faith Hospice has been providing comfort care to those with life-limiting illness or condition as well as support to their families. Our doctors, nurses, social workers, chaplains, hospice aides, and volunteers service patients in a variety of settings. Faith Hospice provides care "wherever our patient puts his/her head down to sleep at night" including patient homes, assisted living or skilled nursing facilities, and hospitals. We also provide end of life care at Trillium Woods, our unique hospice residence in Byron Center. Hospice nursing requires a special gift of caring for those patients and families at such a critical time. We understand that passion and seek to add more health care professionals to our team who share this passion as well!

Faith Hospice seeks a dedicated Manager of Psychosocial and Spiritual Care to lead our team of Hospice Social Workers and Spiritual Caregivers in providing exceptional support to patients and families at the end of life.
What You’ll Do
- Lead & Support – Provide oversight and guidance to our team of Hospice Social Workers and Spiritual Care Chaplains, ensuring high-quality psychosocial and spiritual care.
- Collaborate – Work closely with Clinical Team Managers, the Hospice Residence Manager, and the Quality Coordinator to maintain and exceed care standards.
- Develop & Implement – Oversee the ongoing development of hospice social work and chaplain services, ensuring alignment with best practices and regulatory requirements.
- Evaluate Performance – Participate in employee performance evaluations in partnership with Hospice Clinical Managers.
- Participate in On-Call Leadership – Share in the manager on-call rotation with other Hospice Clinical Managers, providing support as needed.
- Education & Certification:
- Full LMSW licensure in the State of Michigan OR a Master of Divinity with Board Certification as a Chaplain (Clinical Pastoral Education (CPE) is highly valued).
- Experience:
- At least 1-2 years of management experience in a medical or hospice setting.
- Previous hospice experience is highly preferred.
- Skills & Attributes:
- Strong proficiency in electronic medical records (EMR) documentation.
- A leadership style that fosters collaboration, compassion, and excellence in end-of-life care.
- A heartfelt commitment to supporting patients, families, and caregivers through the challenges of hospice care.
