Home Health Aide, Part Time Days

Under the supervision of an RN, the Home Health Aide (HHA) serves as an integral member of the Home Health team, providing personal care and other home services to patients. They perform home visits independently, and are expected to effectively communicate with patients, family members and the Home Health team. The HHA is accountable for scheduling patients assigned to them in a timely manner and managing any cancellations or changes. This position must be able to demonstrate the knowledge and skills necessary to provide patient care and service appropriate to the age of the patients served in their area.

• Provides direct patient care under the general supervision of a nurse or licensed therapist.
• Performs simple procedures as an extension of therapy or nursing services.
• Assists patients in administering medications ordinarily self-administered.
• Recognizes and reports changes in skin condition, body function, and other changes from patient’s baseline.
• Assists patient to maintain adequate nutrition and fluid intake.
• Able to receive written and/or verbal assignments and direction.

• Performs patient care tasks related to activities of daily living to assist the patient in gaining optimal independence in ADLs.
• Accurately completes assigned patient duties within the time frame established by the nurse or therapist.
• Gathers and reports data as directed by the nurse or therapist.
• Recognizes and responds appropriately to patient emergencies

• Utilizes information while upholding the policies and procedures of Holland Hospital to continuously improve patient care, safety, and practice to enhance outcomes.
• Effectively able to communicate with patients, family members, and Home Health staff, either verbally or in writing.
• Reports patient safety issues utilizing the RL occurrence reporting system.
• Able to describe the unit based quality improvement activities and implications for practice. Assists with data collection for quality improvement activities as needed.
• Assists with the identification of actual and potential risks to patient safety.
• Achieves specific initial and ongoing competencies.

• Accurately documents patient care activities and data to ensure visits are billable for reimbursement.
• Requests modifications to the patient care plan as appropriate.
• Documents changes in patient’s condition and ensures these changes are reported to the nurse or therapist in a timely manner

• High School Diploma or GED equivalent required
• Certified Nursing Aide (CNA) required
• BLS for the Healthcare Provider by 60 days after hire required
• Valid MI Driver's License without restrictions
