Job Title: Targeted Case Manager
Division: Behavioral Health Services

Position Summary:
This position is responsible for providing Targeted Case Management Services to referred clients. This position focuses on supporting clients in meeting their treatment goals through a community based, wrap-around approach to care. Collaboration with external stakeholders (e.g., school districts) is an essential aspect of this role.
Essential Job Function:
- Actively adhere to the six core principles of trauma informed care and promotes a trauma informed culture.
- Upholds Holly Hill’s principles and core values
- Responsible for providing Targeted Case Management Services, compliant with regulations set forth by the state of Kentucky
- Responsible for the management of a caseload of 25 referred clients, and meeting agency utilization requirements.
- Responsible for completion of clinical paperwork within agency timelines
- Responsible for active participation in team meetings, staff meetings, and continuing education courses
- Adheres to ethical standards of professionalism including maintaining confidentiality of each client and their family, in compliance with HIPAA regulations and agency policies.
- Prioritizes client care, focusing on delivering quality care for clients
- Seeks to fulfill Holly Hill’s mission by providing quality services to clients served.
- Participates in Holly Hill’s Performance Quality Improvement (PQI) activities as assigned.
- Must be able to drive an automobile; local travel required for this position.
- Have a current/valid driver’s license.
- Have current/valid auto insurance.
Job Requirements:
- Bachelor’s degree in a mental health field with one year of relevant experience
- Must meet Medicaid requirement for Behavioral Health Professional (BHP) or BHP under supervision.
- General knowledge of mental health diagnoses and community resources
- Requires excellent written and oral communication skills.
- Must be able to independently manage time and be flexible.
- Provide treatment services while maintaining clear, professional boundaries.
- Experience working with children and/or families in a mental health setting is preferred
