Job Summary:
The Director of Social Services at Holmes Lake Rehabilitation and Care Center is a key leadership role responsible for overseeing and managing all social service functions within the facility. This role ensures that residents receive comprehensive, compassionate, and personalized support to enhance their quality of life and meet their emotional, social, and psychological needs. The Director will work closely with interdisciplinary teams, families, and community resources to advocate for and support residents.
Work Environment:
Office setting within a skilled nursing facility.
Responsibilities:
- Plan, develop, organize, implement, evaluate, and direct the social service programs.
- Provide information to residents/families as to Medicare/Medicaid, and other financial assistance programs available to the residents.
- Assist the Quality Assessment & Assurance Committee in developing and implementing appropriate plans of action to correct identified deficiencies.
- Assume the authority, responsibility, and accountability of directing the social service department.
Ability to complete daily admissions, MDS submissions in a timely manner.
Set up and hold care plan meetings that are up to state guidelines
Qualifications:
- Minimum of a bachelor’s degree in social work, Psychology, Sociology, Counseling, or related field, and/or NHCA 36-hour Social Work.
- Will maintain required credits in accordance with licensure requirements as needed.
- Must be a minimum of 18 years old.
Skills:
- Proven experience as director of social services.
- In-depth knowledge of long-term care procedures and resident care.
- Great knowledge of legal regulations and best practices in healthcare.
- Excellent ability to lead and develop personnel.
- Great communication and people skills.
