What are the responsibilities and job description for the Community Resource Coordinator position at Holy Cross Health?
General Summary:
This position is a part of a care management team that assists high risk community members and patients in navigating Holy Cross Health's care and community delivery system. It promotes the improvement of the healthcare status through increased health awareness and improved healthy behaviors, amongst Holy Cross Health Center patients and the broader community by facilitating health educational and wellness activities with an emphasis on the importance of screening and early detection, and provides healthcare navigation to connect community members to primary care, health insurance, and other social services as needed.
Supports the Mission, Vision, Values, and Policies/Procedures of Trinity Health and Holy Cross Health.
Minimum Licensure/Certification Required (if applicable):
Basic Life Support (BLS) required
Minimum Knowledge, Skills, and Abilities Required:
- High school diploma or general education degree (GED) Bachelor's degree in public health or related field preferred. 3-5 years of health education and outreach experience required.
- Experience in facilitating/conducting enrollment in available federal, state and local programs to increase access to healthcare and support services (ex.: health coverage via the marketplace, Medicaid, Medicare, food, and housing) .
- Experience in developing and maintaining relationships with community agencies serving target population; participate in community meetings.
- Provide health education on chronic health conditions in a way that promotes understanding and self-management; facilitate screening for early detection of disease; conduct home visits; and link to treatment and care.
- Excellent oral and written communication skills. Bi-lingual in Spanish preferred.
- Evaluate teaching when appropriate (i.e., pre- and post-test, repeat demonstration).
- Experience in collecting relevant data to measure outcomes.
- Computer literate with proficiency in word processing, spreadsheet creation and maintenance (Excel), electronic presentations (Power Point), and Internet software.
- Able to prioritize responsibilities and adapt to changing environments as necessary.
Position Pay range: $21.74-$30.44
Pay is based on experience, skills and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.

Reporting Relationship:
Reports to the manager of community and minority outreach. Under the explicit direction of the manager of community and minority outreach is expected to work independently in accordance with the strategic plan, policies, procedures, performance expectations, and standards of practice. Responsible for reporting and identifying all safety hazards, safety violations, and safety-related incidents to the director and /or safety office or chair of the hospital safety committee. Responsible for strictly adhering to safe work practices, as noted in hospital safety policies and applicable Federal, State, agency and safety-related requirements and regulations.

Salary : $22 - $30
