Interventional Radiology Technician position at Holy Cross Hospital
Work shift 7a-3 : 30p
On call 3 : 30p-7a (weekdays) and 7a-7a (weekends)
On call every other week (10-15 days per month)
Experience in neuro / stroke required.
Job Requirements
BLS certification required for all direct patient care personnel
State License Verification required within 30 days of start
Trinity 9 Panel Drug Screen must be completed within 30 days of start
Trinity Code of Conduct Form must be signed
Trinity I9 must be completed by the agency
Trinity Confidentiality Agreement must be signed
Trinity Integrity and Audit Services Form must be completed
Trinity MVR (Motor Vehicle Report) must be dated within 30 days of start
2 Step PPD OR IGRA (Quantiferon) required
Trinity OFAC must be completed within 30 days of start
COVID-19 Vaccine proof required
MMR vaccination proof required
Hepatitis B vaccination proof required
Influenza Vaccine proof required
Varicella vaccination proof required
Tdap vaccination proof required
Physical / Health Statement required
Trinity Profile including resume and references required
Trinity Conflict Of Interest Disclosure Form must be signed
Trinity Criminal Background Check must be completed
Trinity Education Verification required
Trinity EPLS report must be attached
Trinity GSA / OIG report must be attached
Trinity NSO report must be attached
Trinity System Access Form must be completed
Schedule Information
Work shift from 7 : 00 AM to 3 : 30 PM
On call from 3 : 30 PM to 7 : 00 AM on weekdays
On call from 7 : 00 AM to 7 : 00 AM on weekends
On call every other week, totaling 10-15 days per month
Unit Specific Information
Neuro / stroke experience preferred
Additional Details
American Heart Association certification is required for all direct patient care personnel. Attach scanned copy of certification (with expiration date) to candidate's record in TrinityVMS. Must include an image of the front and back of the card.
