Position Overview:
Provide case management and support through a “housing first approach” for homeless veterans using comprehensive assessment, evaluation, planning and implementation of client’s needs.
Responsibilities:
- Knowledgeable of continuum of care and able to determine appropriateness for CM services.
- Complete initial screening and schedule appointments.
- Complete client intake and program orientation.
- Required intake and orientation documentation and data collection forms are complete, accurate and on time.
- CM needs assessment is thorough, complete and on time. Identifies client strengths, needs, preferences, resources, and barriers.
- Counsel clients on a daily basis regarding housing, life balance, employment and other case management concerns.
- Takes initiative in finding outside resources based on client’s needs.
- Responsible for completing a bio social assessment for clients and interpreting a trauma informed-person centered treatment plan based on individual history and needs.
- Individualized CM goals are based on assessment with respect to client input and client self-assessment. CM plan promotes client knowledge, skills and attitudes that contribute to positive change, and identifies specific CM tasks.
- Re-assess CM plan at regular intervals or when needed by changing circumstances.
- Demonstrates a helping, therapeutic relationship. Able to build rapport and sustain professional relationship.
- Knowledgeable of theories, techniques, research, and best-practice models. Employs methods that have demonstrated effectiveness.
- Adapts strategies to individual characteristics of client and sets appropriate boundaries.
- Documentation is individualized, accurate, complete and on time.
- Accepts responsibility for managing case file from admission through discharge. Ensures all paperwork is accurate, complete and on time.
- Able to determine appropriate funding source and complies with requirements for authorization and fee collection.
- Make and follow up on referrals to other professionals, agencies, community programs or other appropriate resources to meet client needs.
- Effectively advocates on behalf of client. Provides services in the community as needed.
- Complete aftercare plan with client to ensure transition and continued support into next phase.
- Documents CM outcomes using accepted methods and instruments. Participates in data collection for program evaluation.
- Knows, understands, and abides by Agency rules, policies and procedures.
- Helps with housing searches and all tasks related to housing.
- Performs other duties related to the essential duties of the position and within the individual’s capabilities as assigned by one’s supervisor.

Qualifications
- Bachelor’s degree preferred in human services field preferred or equivalent combination of education and experience.
- Maintain valid CPR and First Aid certification.
- Meet State “Good Moral Character” guidelines.
- Valid and Current Driver’s License with acceptable driving record.
Salary : $19 - $22
