A Brief Overview
Clinical Educator is responsible for a variety of quality and educational tasks for Home Care. Identifies issues and works to resolve clinical and utilization issues affecting the service line. Integrates the quality metrics and performance improvement programs within the clinical and financial aspects of the service line. Participates in the planning and coordination of quality and education to include audits for compliance, clinical documentation review and observation of field visits. Participates in the analysis of data regarding trends in utilization, outcomes and quality. Participates in the onboarding & training for new hires and educational activities within the service line.
What you will do
- In collaboration with the Director of Home Care, Home Care Quality Manager & Quality Team, participates in the strategic approach to achieve compliance to the regulatory requirements for Home Care.
- Participate in the review of clinical and financial data to conduct a quantitative analysis on healthcare and quality issues such as utilization, cost and patient experience.
- Perform prebill auditing to ensure compliance with all regulatory bodies, licensing authorities and agency policies.
- Participate in the analysis of data and summarize findings to facilitate the development of education and quality improvement activities to support the service line and maintain the activities as appropriate.
- Establishes mechanisms to identify issues, participate in the development of education to support the service line through the implementation and monitoring of quality improvement activities in compliance with all regulatory bodies, licensing authorities and agency policies.
- Collaborates among the departments within the service line to continually problem solve, seek improvement in processes that affect the quality and delivery of care and assists in the implementation of processes to improve service.
- In collaboration with Quality Team members, ensure compliance among the nursing, rehab and other support staff across the service line.
- Participates in the service line’s performance improvement (PI) program and quality improvement (QI) activities.
- Develops and maintains effective communication with all staff to facilitate continuous readiness and an innovative approach to problem solving.
- Participates in the review & analysis of quality metrics (Star Rating & VBP) and the identification of process changes to improve results.
- Participates in the review & analysis of patient safety & patient satisfaction reports and the identification of process changes to improve results.
- Participates in the review of policies and procedures and the identification of process changes to improve results.
- Participates in the review of the agency's quality management plan and the identification of process changes to improve results.
- Manages annual college clinical rotation assignments / scheduling.
- Manages annual competencies & compliance of annual training.
- Manages onboarding & training the department, as well as management of HR files.
- Perform joint visits across the service line to ensure infection control, patient safety and quality care.
- Manages CMS and insurance requests and appeals.
- Assist with Quality Manager & Clinical Supervisor Task in their absence.
- Other duties as assigned.
