Our guiding principles of independence, dignity, and quality of life are realized through our interconnected core service areas: nutrition, transportation, home repair, and social services. We are committed to building an inclusive workplace and offer you the opportunity to work to your fullest potential while making a difference in the community.

Summary:

Homage’s Home Modification program is hiring a Licensed Occupational Therapist to conduct in-home client assessments for older adults in Snohomish County. The program’s goal is to create a safe home environment and help clients participate fully in their daily activities. The OT will visit clients (aged 62 ) on two separate occasions to: 1) assess the client's primary residence for home modifications to meet their specific needs and goals and create a report to the Home Modification office which will prepare a work order; and 2) make a final assessment to ensure the work is completed satisfactorily. Additional duties may be assigned, as needed.

Essential Duties and Responsibilities:

  • Conduct in-home client visits using an aging-in-place assessment to evaluate functional abilities and the home environment. Recommend cost-effective, high-impact modifications and adaptive equipment to enhance safety and independence

  • Educate clients on the relationship between home accessibility modifications and overall health and safety

  • Identify and report clients experiencing significant memory loss that may compromise their ability to live alone, connecting them with appropriate resources at Homage and beyond
