Job Title: Nursing Care Coordinator (LPN/RN)
Job Summary: The Nursing Care Coordinator is responsible for overseeing and coordinating the delivery of high-quality nursing care to clients in a private duty nursing setting. This role involves reviewing nursing notes, scheduling nurses and aides, and participating in an on-call rotation to ensure continuous care and support for clients.
Key Responsibilities:
- Review Nursing Notes: Regularly review and assess nursing notes to ensure accuracy, completeness, and compliance with company policies and regulatory standards.
- Scheduling: Coordinate and manage the schedules of nurses and aides to ensure adequate coverage and continuity of care for all clients.
- On-Call Rotation: Participate in an on-call rotation to provide support and address any urgent client needs or staffing issues outside of regular business hours.
- Client Care Coordination: Develop and implement individualized care plans for clients, ensuring that all aspects of their care are addressed and managed effectively.
- Communication: Serve as a liaison between clients, families, healthcare providers, and staff to facilitate clear and effective communication regarding client care.
- Training and Support: Provide training, guidance, and support to nursing staff and aides to ensure they are well-prepared to deliver high-quality care.
- Quality Assurance: Monitor and evaluate the quality of care provided, identifying areas for improvement and implementing corrective actions as needed.
- Documentation: Maintain accurate and up-to-date client records, including care plans, progress notes, and other relevant documentation.
- Other Duties as Assigned: Perform additional tasks and responsibilities as required to support the overall operations and objectives of the company.
Qualifications:
- Education: Licensed Practical Nurse (LPN) or Registered Nurse (RN) with a valid state license.
- Skills: Strong organizational, communication, and leadership skills. Proficiency in electronic health records (EHR) systems is preferred.
- Attributes: Compassionate, detail-oriented, and able to work effectively in a fast-paced environment.
Working Conditions:
- Schedule: Full-time position with participation in an on-call rotation.
- Location: Primarily office-based with occasional home visits to clients as needed.
