Essential Duties and Responsibilities:
- Perform related duties as assigned by the supervisor.
- Maintain compliance with all company policies and procedures.
- Do all assessments for admissions and reassessments.
- Do all initial care plans and update care plans as necessary.
- Do supervisory visits at least every 90 days.
- Be a Liaison between clients, their families, and other agencies as necessary.
- Oversee and supervise all direct care staff (LPNs, CNAs, DSPs and PCAs)
- Communicate with the personnel manager for personnel issues.
- Give guidance for staff development.
- Perform PCA/DSP classes.
- Keep up to date with in-home care regulations for state and federal.
- Assist with answering office phones.
- Carry on-call phone at least once a month.
- Track reassessment, care plans, and supervisory visits.
- Perform well checks of clients as necessary.
- Do 225 admissions and discharges.
- Review care plans with clients and their families upon admissions and reassessments.
- Ensure care plans are in the client’s homes.
- Ensure DMAS forms are filled out correctly.
- Do TB Screenings on hire.
- Do authorizations in Kepro for state clients.
Education and/or Work Experience Requirements:
- Excellent verbal and written communication skills.
- Excellent computer proficiency (MS Office – Word, Excel, and Outlook)
- Must be able to work under pressure and meet deadlines, while maintaining a positive attitude and providing exemplary customer service
- Ability to work independently and to carry out assignments to completion within parameters of instructions given prescribed routines, and standard accepted practices.
- High school diploma or GED required.
- Must have at least 1 year of experience in related field
- Current RN Licensed
Physical Requirements:
- Ability to perform essential job functions consistently safely and successfully with federal, state, and local standards.
- Ability to maintain regular, punctual attendance consistent with federal, state, and local standards.
- Must be able to lift and carry up to 50 lbs
- Must be able to talk, listen and speak clearly on the telephone.
