What are the responsibilities and job description for the RN Client Care Coordinator position at Home Care Assistance?
In this unique dual role position, you will act as the Home Care Assistance of Registered Nurse Case Manager and you will represent HCA in the community, with case managers, rehab centers, and senior living facilities. This is an excellent position for a seasoned registered nurse with an outstanding income opportunity. All of our cases are private pay, so you will be working in some of Virginia's finest homes throughout Hampton roads and the surrounding areas.
As our RN Case Manager you will:
- Manage the care for our in-home care clients throughout Richmond and the surrounding areas.
- Conduct in-home client care need assessments and develop customized client care plans.
- Provide in-home supervision and client specific training for caregiver teams on the implementation of client specific care plans.
- Solve problems related to hospital to home transitions or specific client care needs.
- Represent HCA and education case managers, rehab centers, senior living facilities and the Richmond senior care community about the benefits of our high quality service.
Benefits of this full time position Include:
- $40 an hour including time spent traveling to clients homes.
- Typically you will work approximately 20 hours a week, with a flexible work schedule and desirable daytime weekday working hours - typically between 9 - 4 Monday through Friday.

The right individual for this position will possess the following qualities:
- Must possess a positive service based attitude and a genuine desire to improve the quality of senior’s lives.
- The ability to work effectively with a wide variety of home care situations, clients and employees.
- Excellent interpersonal communication, presentation and relationship building skills.
- A current valid Virginia Register Nurse license, Virginia driver's license, a good driving record, and able to pass a Virginia State Police background check.
- Experience in senior care with established relationships is preferred
