We are hiring for the important job of LPN Intake / Scheduler supporting both Medicare and our Private Duty business lines working at our Corporate Office location in Clearwater M-F (this is NOT a remote position). As a locally owned and fast growing home health agency, we are excited to hire an experienced scheduler / intake person that is looking to make a positive change in their career.
Home Care Now's mission is to improve our client's lives through the delivery of exceptional service and our continual improvement of their home health experience. This position is an integral part of our company's success serving the vulnerable members of our community. As an LPN Intake / Scheduler, you will be responsible for new client intakes, scheduling private duty shifts and Medicare visits in accordance with physician orders. You will participate in the recruiting and hiring activities, assign personnel to shifts and visits, ensure compliance with order frequencies, and match caregivers to our clients. You may participate in the on call rotation with your other team members.
Additional Responsibilities :
- Receives and coordinates home care referrals and / or staffing requests
- Assigns qualified and competent personnel to the case
- Ensures orders are timely and filing is current
- Supports agency recruitment efforts to include interviewing, hiring and orienting field staff
- Upholds agency values and supports our mission statement
- Adhers to Medicare and Florida ACHA home health rules and regulations
- All other duties as assigned
