The Registered Nurse (RN) in Home Health delivers and oversees patient care in their homes as prescribed by the physician and adheres to applicable laws, regulations, and agency policies.
This professional coordinates the total plan of care with other healthcare providers involved in the patient's care, ensuring continuity by planning and exchanging information with the physician, agency personnel, patient, family, and community resources within a Point of Care setting.
This nurse makes the initial and/or comprehensive nursing evaluation visit, determines the primary focus of care, develops the plan of care within state-specific guidelines, and submits accurate, complete, and timely documentation in collaboration with physician orders.
This RN performs patient comprehensive assessments at designated time points and develops an appropriate plan of care.
This professional ensures patients meet home health eligibility and medical necessity guidelines as defined by payer sources.
Requirements
Experience
- Prior to making a psychiatric nurse visit, adhere to Psychiatric Skilled Nursing/Scope of Care.
- Must have experience within the last five years. If not, provide documentation supporting psychiatric retraining classes or CEUs to update psychiatric knowledge.
License
- Must have current RN licensure.
- Current CPR certification is required.
- Must have a current driver's license and vehicle insurance, and access to a dependable vehicle, or public transportation.
