The Registered Nurse provides and directs patient care in their homes according to physician orders and applicable laws. They coordinate care with other healthcare professionals, achieving continuity by planning and exchanging information with the physician, agency personnel, patient, family, and community resources.
The nurse makes initial and comprehensive assessments, develops plans of care within state-specific guidelines, and submits accurate documentation in collaboration with physician orders. They ensure patients meet home health eligibility and medical necessity guidelines as defined by payer sources.
Experience Requirements
- The agency adheres to Psychiatric Skilled Nursing/Scope of Care before the RN makes a psychiatric nurse visit.
- The RN must have experience within the last five years or documentation supporting psychiatric retraining classes or CEUs to update psychiatric knowledge.
License Requirements
- The RN must have current licensure.
- A current CPR certification is required.
- A current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation are necessary.
