Job Description
The Registered Nurse (RN) in Home Health provides and directs provisions of nursing care to patients in their homes as prescribed by the physician and in compliance with applicable laws, regulations, and agency policies. Your primary focus will be on coordinating total plan of care with other health care professionals involved in care and helping to achieve and maintain continuity of patient care.
Key Responsibilities:
- Conduct initial and comprehensive nursing evaluations to determine primary focus of care
- Develop and implement individualized plans of care within state-specific guidelines
- Ensure accurate, complete, and timely documentation in collaboration with physician orders
- Collaborate with healthcare professionals to achieve optimal patient outcomes
Requirements
To succeed in this role, you will need:
- Current RN licensure
- Current CPR certification
- A reliable vehicle and valid driver's license (if driving is required)
