The Registered Nurse (RN) in Home Health plays a vital role in providing and directing nursing care to patients in their homes, as prescribed by physicians and in compliance with applicable laws, regulations, and agency policies.
Responsibilities include coordinating total plans of care with other healthcare professionals, achieving and maintaining continuity of patient care, and submitting accurate documentation in collaboration with physician orders.
You will make initial and comprehensive nursing evaluation visits, determine primary focus of care, develop plans within state-specific guidelines, and ensure patients meet home health eligibility and medical necessity guidelines.
To be successful in this role, you must have prior experience within the last five years or documentation supporting psychiatric retraining classes or CEUs to update psychiatric knowledge.
Requirements include current RN licensure, CPR certification, a valid driver's license, vehicle insurance, and access to reliable transportation or public transportation.
