Job Description
The Registered Nurse (RN) in Home Health is responsible for providing and directing provisions of nursing care to patients in their homes, as prescribed by the physician and in compliance with applicable laws, regulations, and agency policies.
- Leadership Skills: Demonstrates leadership skills by guiding and mentoring team members, promoting a culture of caring and respect within the team.
Key Responsibilities
- Coordinates total plan of care with other healthcare professionals involved in care, ensuring continuity of patient care by planning and exchanging information with physicians, agency personnel, patients, family, and community resources.
- Develops and implements comprehensive nursing plans of care that address patients' physical, emotional, and social needs.
- Cultivates relationships with physicians, therapists, and other healthcare professionals to ensure seamless care transitions and optimal patient outcomes.
Requirements
- Current RN licensure in state of practice
- Current CPR certification
- Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation
