Job Summary

We are seeking a skilled Registered Nurse (RN) to provide and direct nursing care to patients in their homes, as prescribed by physicians and in compliance with applicable laws, regulations, and agency policies.

• Coordinates total plan of care with other healthcare professionals involved in patient care.
• Achieves continuity of patient care by planning and exchanging information with the physician, agency personnel, patient, family, and community resources.

Responsibilities

1. Conducts initial and comprehensive nursing evaluation visits to determine primary focus of care and develops a plan of care within state-specific guidelines.
