Job Description
The Registered Nurse (RN) in Home Health plays a critical role in providing and directing provisions of nursing care to patients in their homes, as prescribed by the physician and in compliance with applicable laws, regulations, and agency policies.
- Patient-Centered Care: Prioritizes patients' needs, preferences, and values in all aspects of care, ensuring that each patient receives personalized attention and support.
Key Responsibilities
- Conducts initial and comprehensive nursing evaluations to determine primary focus of care and develops the plan of care within state-specific guidelines.
- Develops and implements individualized plans of care that address patients' physical, emotional, and social needs.
- Collaborates with physicians, therapists, and other healthcare professionals to ensure seamless care transitions and optimal patient outcomes.
Requirements
- Current RN licensure in state of practice
- Current CPR certification
- Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation
