Job Description:
The Registered Nurse (RN) in Home Health provides nursing care to patients in their homes as prescribed by the physician. You will coordinate with other healthcare professionals to achieve continuity of patient care and make sure that patients meet home health eligibility and medical necessity guidelines.
Responsibilities:
- Makes initial and comprehensive nursing evaluation visits
- Determines primary focus of care and develops a plan within state-specific guidelines
- Ensures accurate, complete, and timely documentation
- Coordinates with the physician, agency personnel, patient, family, and community resources
- Performs patient comprehensive assessments at designated time points
Requirements:
- Current RN licensure required
- Current CPR certification required
- Must have a current driver's license and vehicle insurance
- Must have experience within the last five years as a psychiatric nurse or have completed psychiatric retraining classes/CEUs
