$35 - $45 hourly

• Administers medications and treatments as prescribed and according to policy
• Assesses patients' conditions, documents changes, and reports them to the supervisor, physician, and family
• Provides direct and individualized nursing care to clients based on nursing standards and under supervision
• Implements plans of care formulated by the physician and Registered Nurse
• Ensures quality and safety of home health services to patients and families
• Educates patients and their caregivers on pertinent information related to diagnosis and plan of care
• Maintains a safe and healthy work environment
• Coordinates home health services with Registered Nurses and clinical staff
• Assists clients in learning appropriate self-care activities
• Monitors and documents activity as required
• Performs other related duties as assigned by management
• Administering IVs and managing dressings to promote healing
• Ensuring clear communication with the patient's medical provider
• Assisting with medication administration and wound care
• Providing support and guidance during the healing process
• Catering to the specific needs of each patient based on their age, injury, or illness

• Records and reports all relevant activities in accordance with the plan of care
• Delivers education and information applicable to the diagnosis and plan of care to patients, clients, and their families/caregivers
• Uses equipment and supplies in an effective and efficient manner
• Documents and reports activities as per requirements
