Canyon Home Care & Hospice is looking for a compassionate and enthusiastic Physical Therapist to join our Home Care & Hospice team. Evaluate Patients and Create a Plan of Care
Our Physical Therapists evaluate and assess our patients while organizing a Plan of Care with specific goals to assist with patient recovery.
Assist and Teach Patients
Therapists assist patients with their therapy program during visits and teach patients how to perform a successful at-home therapy program during non-visit time.
Re-Evaluate and Revise Plan of Care
Therapists re-evaluate and revise the patient Plan of Care as necessary and document clinical progress notes on the day of service. Therapists also participate in case conferences and act as case managers when given permission.
Provide Supervision for Other Caregivers
Physical Therapists instruct and supervise other team caregivers in appropriate patient care.
****MUST HAVE AN ACTIVE STATE PHYSICAL THERAPIST LICENSE****
Previous home health experience is preferred, but not necessary. Reliable transportation is required.

- Candidate must be able to evaluate the mental, emotional and physical status of patients.
- Candidate must understand federal, state and local healthcare standards and regulations.
- Candidate must possess current state licensure and CPR certification.
- Candidate must possess a basic understanding of technology including the operation of tablets and computer applications.
