Registered Nurse Case Manager
We are a Central Coast focused hospice agency looking for a compassionate and dependable Registered Nurse to join our team. We are looking for individuals that understand the value of alleviating pain and suffering at end-of-life while meeting a shared goal of helping individuals live their final days with dignity and comfort. We currently have a need for a RN Case Manager to support our growing program in Santa Cruz County.

Although this position is Monday through Friday from 8:30 - 5pm, our RN Case Manager role is autonomous with built in flexibility based on your patients’ needs. Required License and Qualifications:
Registered nurse with current CA license
Maintains current CPR Certification that meets American heart Association BLS Guidelines
Licensed driver with automobile that is insured in accordance with state requirements
Able to lift, position, and/or transfer patients
Essential Job Functions / Responsibilities
Provide in-home and facility visits to assess patient eligibility and needs to initiate appropriate interventions
Develops care plan and teaches patient and caregivers care techniques as appropriate
Manage and administer medications as appropriate
Educate and prepare patients, families and caregivers to end-of-life processes
Provide mental and emotional support to patients and families as appropriate
Lead and collaborate with other members of inter-disciplinary team (Social Worker, Chaplain, CNA) providing patient care in conjunction with Hospice Medical Director
Collaborate and communicate with referral partners as appropriate
Complete accurate and relevant clinical notes in EMR
