This role involves working closely with the patients, families, and healthcare professionals. The ideal candidate will have excellent communication and organizational skills, as well as a compassionate approach to patient care.
The Home Care Coordinator will assist our patients by matching them with suitable home health aides to ensure satisfaction with their home care services. The primary goal is to ensure that each patient receiving care is paired with a quality home health aide to care for them long term.
Responsibilities:
- Serve as the primary point of contact for the patients and their families, addressing any concerns or questions.
- Facilitate communication between patients, families, and caregivers to ensure continuity of care.
- Manage telephone calls in a supportive, reassuring manner for patients, families, and healthcare professionals.
- Schedule and coordinate home care services, ensuring timely and efficient delivery of care.
- Maintain a schedule of all services per month.
- Verify attendance.
- Submit time sheets accurately in a timely manner for payroll purposes.
- Cover open cases as they occur.
- Locate coverage for open cases created by call outs.
- Communicate weekly regarding scheduling, availability, attendance, and assignment.
- Complete pre-billing on a weekly basis.
- Document all covered cases and open cases in the scheduling system in real-time.
- Maintain accurate and up-to-date patient records and documentation.
- Assist with the screening of caregivers.
- Manage patient intake and discharge processes.
- Handle scheduling, billing, and other administrative tasks as needed.
- Develop and implement strategies for retention.
- Maintain patient confidentiality and privacy.
- Ensure compliance with company policies and procedure
Skills:
- Bilingual for any of the following: English/Spanish ; English/Creole ; English/Urdu
- Previous scheduler experience is preferred
- Strong organizational and time-management skills.
- Excellent communication and interpersonal skills.
- Ability to work independent and as part of a team.
- Ability to handle a fast-paced environment and deadlines
- Strong phone customer service skills
- Proficiency in Microsoft Office and home care management software.

Pay: $24.00 - $30.00 per hour
