OASIS PT Responsibilities :
- Perform Admission / "Start of Care" OASIS Visits throughout an area of the Metro
- Evaluate and assess clients for home health services and create their plan of care
- Recommend services from other disciplines that have not yet been ordered by the physician
- Collaborate with clients, families, physicians, and other members of the home care team
