Introduction to the Company
Home & Health Care Management is the oldest home health care agency in Northern California. Our agency specializes in delivering comprehensive in-home care and case management for older adults, children, and those living with disabilities across many counties. We use a matrix of Medi-Cal, Medicare and targeted health programs to find solutions that work for our clients.
Overview of the Role
We are looking for a Full-time Registered Nurse to work with a dedicated interdisciplinary health team to provide case management services for HIV/AIDS Clients in our area.
Duties:
- Determine program eligibility
- Establish and maintain a Patient centered care plan
- Coordinate care needs with the Clients’ medical Providers
- Provide monthly service plan monitoring with the assistance of the Agency Social Worker
- Attend and document Interdisciplinary team conference meetings monthly
- Provide comprehensive case management
- Complete Reassessments every 180 days
- Perform quality assurance and performance improvement activities to improve Client outcomes and assure compliance with Agency requirements and State regulations.
Essential Requirements:
- Licensed as a Registered Nurse in the State of California
- One year experience working in community health setting, a home health agency or in a hospital; or have a baccalaureate degree in nursing or program flex as approved by the Dept. of Public Health Licensing and Certification. (Agency will apply on your behalf)
- Valid California Driver’s License
- Eligible to be insured under our liability policy with a clean DMV report
- Your own vehicle and proof of current auto insurance
- Comfortable using computers for day-to-day documentation in our electronic health record. Ability to work well in an interdisciplinary team, is able to independently set a schedule and maintain deadlines and reporting requirements. Excellent verbal and written communication and critical thinking skills
Prior experience with HIV/AIDS Clients if preferred but not necessary.
Service Region/ Location:
This is not a remote role and requires you to work in our Chico office.
Schedule:
This role is full-time, 40 hours per week, from 8am to 5pm Monday through Friday with an hour for lunch from 12pm-1pm. No weekend work is required.
Travel:
Home health care requires you to drive to patient’s homes. You will need to be in the field approximately 50% of your time, and in the office for the other 50% of your time, depending on caseload needs.
Physical Requirements:
Conditional upon job offer is successfully passing a medical exam that tests amongst other things the ability to move, visual acuity, and hearing acuity. Physical requirement for this role specifically include
- Sits, stands, bends, lifts, walks, and moves intermittently during working hours
- Able to lift to 35 pounds without assistance
- Able to drive intermittently
Compensation:
Pay range is $43 to $50 per hour. Exact compensation is based on skills and experience.
Benefits
You will be reimbursed for the mileage you travel from the office to participant’s homes.
You will receive a monthly phone stipend.
On the first of the month following 60 days of employment you will be eligible for health insurance, dental insurance, vision insurance, life insurance, as well as Flexible Spending Account (FSA).
Leave accrues starting on your first day of employment, and you can take paid vacation, sick and holiday leave after 90 days.
