Bachelor or Mastered Prepared Social Worker BSW/MSW is wanted to join an interdisciplinary team and innovative agency that creates new programs and services to meet the needs of our North State community. Our Waiver program has been providing HIV/AIDS services since 1996. This position provides home and community based case management services in conjunction with a RN to HIV clients under the Medi-Cal AIDS Waiver Program.

Duties:
Duties include psycho-social assessments, creating patient centered care plans, making home visits at least every 90 days and/or phone calls at least monthly to clients discussing options for community resources for housing, MediCare/Cal , IHSS, VA, as well as medical and behavior health care.
You will have the time needed to manage your caseload of approximately 55 clients. Attend case conference with other Health care providers and document your care.
You will be trained and be paired with nurses and other social workers as you learn the role and become familiar with the documentation guidelines for the various HIV programs. Programs cover clients who reside in one of our eight county catchment area - Butte, Glenn, Yuba, Sutter, Colusa, Tehama, Shasta and Trinity.
Essential Requirements:
- Bachelor’s degree in social science
- Valid California Driver’s License
- Eligible to be insured under our liability policy with a clean DMV report
- Your own vehicle and proof of current auto insurance
- Comfortable using computers for day-to-day documentation in our electronic health record.
- Ability to work well in an interdisciplinary team, is able to independently set a schedule and maintain deadlines and reporting requirements.
- Excellent verbal and written communication and critical thinking skills.
Service Region/ Location:
This is not a remote role and requires you to work in our Chico office.
Schedule:
This role is full-time, 40 hours per week, from 8am to 5pm Monday through Friday with an hour for lunch from 12pm-1pm. No weekend work is required.
Travel:
Home health care requires you to drive to patient’s homes. You will need to be in the field approximately 50% of your time, and in the office for the other 50% of your time, depending on caseload needs.
Physical Requirements:
Conditional upon job offer is successfully passing a medical exam that tests amongst other things the ability to move, visual acuity, and hearing acuity. Physical requirement for this role specifically include
- Sits, stands, bends, lifts, walks, and moves intermittently during working hours
- Able to lift to 35 pounds without assistance
- Able to drive intermittently
Compensation:
Pay range is $22 to $30 per hour. Exact compensation may vary based on skills and experience.
Benefits
You will be reimbursed for the mileage you travel from the office to participant’s homes.
You will receive a monthly phone stipend.
On the first of the month following 60 days of employment you will be eligible for health insurance, dental insurance, vision insurance, life insurance, as well as Flexible Spending Account (FSA).
Leave accrues starting on your first day of employment, and you can take paid vacation, sick and holiday leave after 90 days.
