Job Summary
We are seeking a compassionate and dedicated Home health care staff, PHYSICAL THERAPISTS, PHYSICAL THERAPIST AIDES, HHA’s, LVNs to provide high-quality care to our clients in their homes. The ideal candidate will have experience in senior care and care for individuals in need of home health care services, with a strong understanding of medical documentation and compliance with HIPAA regulations. This role requires a commitment to enhancing the quality of life for individuals, particularly those dealing with acute or chronic conditions, post operative therapy and care, assistance with ADLs, pain management, medication reconciliation and teachings, and overall care for those individuals in need in their home setting .
Duties
- Provide direct care to clients in their homes, ensuring adherence to individualized care plans.
- Assist clients with daily living activities, including bathing, dressing, grooming, and mobility support.
- Administer medications and monitor vital signs as prescribed by healthcare providers.
- Maintain accurate medical documentation using paper/electronic system charting while ensuring compliance with HIPAA standards.
- Collaborate with family members and healthcare professionals to develop and update care plans tailored to each client's needs.
- Educate clients and their families on health management and safety practices within the home setting.
- Lift and transfer clients safely using appropriate techniques and equipment.
Requirements
- Valid nursing license (LPN or RN) in the state of practice.
- Experience in senior care at home settings is preferred.
- Abilities to perform wound cwith supplies provided By agency (sterile, and non-sterile technique)
- Strong communication skills, both verbal and written, for effective interaction with clients and families.
- Proficiency in medical documentation systems and adherence to HIPAA regulations.
- Ability to lift patients safely and perform physical tasks associated with Range of motion exercises, gait training, and strengthening.
- Compassionate demeanor with a genuine desire to help others improve their quality of life.
-**** Ability to chart electronically for patient visits using software provided, RNs to be able to complete OASIS for each patient assessment electronically. (Training/education available if needed) *****
-****willing to do visits up to 60 miles distances IF NEEDED. (EXTRA PAY ACCOUNTED FOR LONG DISTANCES!)
