- Complete visits and directs patient care and consultation services for hospice and palliative care programs; may include visits to include but not limited to homes, nursing facilities, hospitals and community hospice house.
- Provides expertise regarding pain and symptom management to all referred patients.
- Completes consults and patient evaluations in timely manner and codes appropriately for reimbursement purposes, with adequate clinical resources to cover for these clinical services while maintaining productivity standards.
- May serve as backup to attending physician for patients presenting to a Hospice program team without a local physician, and directs patient’s plan of care.
- Assists the Hospice and Palliative Care Medical Director in developing, maintaining, and monitoring all medical standards and protocols for the hospice and palliative care program.
- Acts as a resource to community physicians and clinicians regarding symptom management.
- Responsible for on-call rotation for Community Hospice House and hospice program.
- Maintains expertise through attendance at seminars, conferences and in-services related to hospice and/or palliative care programs
- Provides palliative care in the community, intermittent patient care at Hospice house, care of hospice patients, once a weekday night call and call every 5th weekend.
- Masters or post graduate degree as required to obtain a Nurse Practitioner license.
- Valid licenses as Registered Nurse (“RN”) and Advanced Practice Registered Nurse ("APRN") in NH.
- Unrestricted DEA provider registration and Controlled Substance License.
- Eligible to obtain hospital privileges.
- Eligible for the Advanced Hospice and Palliative Care Certification.
- Excellent communication, presentation and teaching skills.
- Local Hospital privileges at the hospital(s) high in referral volume.
- Comprehensive knowledge of palliative care with special expertise in pain and symptom control and related pharmacology.
- Certified in Advanced Hospice and Palliative Care.
- Demonstrates expertise in the psycho‑social aspects of the care of the terminally ill patients and their families.
- EMR software proficiency.
- Registered as a APRN with New Hampshire’s Board of Nursing.
- BLS/CPR Certification.
- Satisfactory criminal record.
- Satisfactory motor vehicle record.
- Active auto insurance listing employee as insured operator.
