Responsibilities
- Implements physical therapy treatment plans in consultation with supervising physical therapist.
- Reinforces therapeutic plan following discharge by instructing patients, families, and caregivers in home exercise programs.
- Communicates need for changes in patients goals to supervising physical therapist.
- Effectively communicates all aspects of patient care to appropriate team members; contacting team manager as needed.
- Communicates patient’s progress or need for discharge to supervising physical therapist.
- Takes lead role in educating patient and caregivers on proper body mechanics.
- Documents in patient electronic records by assigned deadlines.
Qualifications
- Associate’s degree in Physical Therapy.
- Registered as Physical Therapy Assistant with New Hampshire’s Board of Nursing.
- A minimum of two years therapy assistant experience, either hospital or clinic experience.
Preferred Qualifications
- Homecare Experience.
Required
- Registered as Physical Therapist Assistant with New Hampshire’s Board of Nursing.
- CPR Certification.
- Satisfactory criminal record.
- Satisfactory motor vehicle record, reliable vehicle and auto insurance.
- Must be able to work one weekend a month and share in holiday rotation working one winter and one summer holiday a year.
