Service Areas: Nursing home/facilities in Southern New Hampshire greater Merrimack, Nashua, Manchester communities.
Responsibilities
- Completes an initial home health assessment of patient and family to determine hospice care needs. Provides a complete physical assessment and history of current and previous illness(es).
- Utilizes the nursing process in planning, providing and evaluating skilled needs.
- Completes a comprehensive patient assessment.
- Develops a care plan, which establishes goals based on nursing diagnosis and incorporates therapeutic, preventive, and rehabilitative nursing actions. Includes the patient and family on the planning and revising process.
Qualifications
- Graduate of an accredited school of nursing.
- Associate degree in nursing.
- Registered as RN with New Hampshire’s Board of Nursing.
- A minimum of one year recent acute care experience.
- Ability to exercise initiative and independent judgment.
- EPIC experience preferred, willing to train!
Preferred Qualifications
- Bachelor degree in nursing.
- Community Health/Education experience.
- Certification in Hospice and Palliative Care, Oncology or end-of-life experience is preferred, but willing to train the right candidate.
- EMR software proficiency.
Required
- Registered as a RN with New Hampshire’s Board of Nursing.
- CPR Certification.
- Satisfactory criminal record.
- Satisfactory motor vehicle record.
- Work one weekend a month pays additional $4.00 an hour and agency holidays 1 winter and 1 summer required pays time and a half.
