What are the responsibilities and job description for the Triage RN Overnight - Homecare position at Home Health & Hospice Care?
Are you an RN who has family obligations during the day, wanting to get back to patient care, who is able to take patients calls, assess their clinical needs and if needed able to go out to their home during the hours from 5pm to 8am? We are looking for a full time Triage RN to work three nights a week Wed., Thurs., Fri.'s and share in our agency holiday rotations. Position will cover a medical leave part time and will transition to per diem. This position could be ideal for the right person!
You must be able to go to the patients home as needed during the hours of 5pm to 8am. While the majority of calls can be handled over the phone there are times when the patient will need to see you! Our 3 month orientation / training is based on one's ability to manage the acute care needs of our patients. Initial training will be weekdays.

Responsibilities
- Triages patient calls in effort to assist virtually.
- If necessary goes to patient's home as needed to provide care.
- Completes home health assessment of patient. Provides physical assessment and care, documents utilizing EPIC our EMR.
- Utilizes the nursing process in planning, providing and evaluating skilled needs.
- Assists with care plan, which establishes goals based on nursing diagnosis and incorporates therapeutic, preventive, and rehabilitative nursing actions. Includes the patient and family on the planning and revising process.
Qualifications
- Graduate of an accredited school of nursing.
- Associate degree in nursing.
- A minimum of two years’ recent acute care experience.
- Ability to exercise initiative and independent judgment.
Preferred Qualifications
- Bachelor’s degree in nursing.
- One to two years home health experience.
- Community Health/Education experience.
- Certifications: IV, Chemotherapy, Ostomy, as examples.
- Familiarity with OASIS and what drives reimbursement.
- EMR software proficiency.
Required
- Registered as a RN with New Hampshire’s Board of Nursing.
- CPR Certification.
- Satisfactory criminal record.
- Satisfactory motor vehicle record.
