What are the responsibilities and job description for the Wound Care Coordinator- Field Technician position at Home Health & Hospice Care?
Qualifications: Minimum Qualifications
- Graduate from an accredited school of Nursing, OT, or PT with Bachelor’s degree.
- Current NH licensure as RN, PT or OT.
- Current NH licensed clinician certified in wound care.
- Minimum of 2 years experience in wound care, preferably in home health.
- Strong knowledge of wound care treatments, wound healing processes, and pressure injury prevention.
- Excellent clinical assessment and critical thinking skills.
- Strong organizational and communication skills.
- Proficiency in electronic medical records and OASIS documentation related to wounds.
- Strong knowledge of insurance regulations related to wound care documentation.
- Strong knowledge of supply management/formulary management.
- Ability to educate peers related to wound care and documentation.
- Knowledge of treatment of wounds that align with hospice goals of care.
Other Required
- CPR certified
- Satisfactory Criminal record and BEAS Record
- Current state driver’s license required.

- Responsibilities:
- Direct Clinical (in the field) – 80% of working hours (32 hours):
- Assesses, develops, implements, and evaluates individualized wound care treatment plans for home health patients
- Performs advanced wound care in the home
- Conducts in-home visits to assess and manage complex wound cases.
- Monitors and tracks wound healing progress and makes suggestions for treatment adjustments as needed.
- Participates in teams/case conferences to discuss wound management.
- Ensures cost effective use of wound care supplies while maintaining quality patient outcomes.
- Direct Clinical (in the field) – 80% of working hours (32 hours):
2. Coordination of Wound Care Program - (in office) – 20% of working hours (8 hours):
- Provides education to clinicians, patients, and caregivers on wound prevention, healing, and management.
- Collaborates with providers and clinics to ensure optimal patient outcomes.
- Provides ongoing education, training, and support to nursing staff on best wound care practices, documentation, and treatment modalities.
- Serves as a clinical resource for staff regarding wound management techniques and protocols.
- Develops and implements competency assessments for wound care skills among clinical staff.
- Assists in ensuring all wound care documentation meets regulatory and agency standards.
- Monitors trends in wound care outcomes and assists with quality improvement initiatives related to wound care.
- Stays up to date with all regulatory requirements for wound care in home health.
