Responsibilities:
- Provide personal care services to patients in accordance with their individual care plans
- Assist with activities of daily living, such as bathing, dressing, grooming, and toileting
- Observe and report changes in the patient's condition to the healthcare team
- Maintain a safe and clean environment for the patient
Requirements:
- Certified Nursing Assistant (CNA) or Home Health Aide (HHA) certification
- Prior experience in home health care or a related field is preferred
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
- Own car, valid driver's license and auto insurance
