What are the responsibilities and job description for the Clinical Nurse Supervisor position at Home Health?
Home Health is seeking an experienced LVN/RN for office and field visits in Fort Worth-Dallas area. Ideal candidate will have at least 2 years of recent home health experience. Must have clinical assessment skills and knowledge of OASIS D1 and QA.

About the Position:
Registered Nurse is responsible for comprehensive assessments in the patient's home, including the completion of OASIS using electronic documentation (Axxess).
- Evaluate patient and home health environment to determine required services and involve the patient and patient's family in developing the plan of care.
- Develop the POC based on findings from evaluation visit.
- Serve as the patient's advocate and maintain ongoing communication with physician, referral sources, caregivers, and facility management to facilitate coordination of care.
- Administer medications and treatments as prescribed by physician
- Provide direct supervision of nursing clinical care team members (LVN's & HHA's).
- Participate in regular case conference and in-services
For Full Time Hours -40 hours/week, combined office and visits.

Requirements
- At least 2 years of previous adult home health experience required,
- Current Texas Registered Nurse (RN) license
- 5 years of clinical nursing experience
- Current Texas driver's license
- CPR Required
- At least 2 years of previous OASIS experience and QA experience
