What are the responsibilities and job description for the Healthy Starts RN - Peterborough position at Home Healthcare and Hospice Community Services?
Our Healthy Starts program is looking for a Registered Nurse to join their team. This position would service the Peterborough area, working on a variable hour basis. The Healthy Starts RN would provide direct nursing care to clients including assessment, plan of care, care management, education, counseling and home visits. Assure that the multi-faceted physical, psychosocial and educational needs of each client are met. Facilitate interagency action, communication and referral as needed. Increase awareness of the target population and general public regarding the availability of the program.

Requirements:- Associates degree in nursing with a minimum of two (2) years experience in maternal child health nursing; or
- A combination of education and experience that is deemed to be equivalent to the above noted educational and experience requirements;
- Possession of a current State of New Hampshire license to practice as a registered nurse. Maintain current cardiopulmonary resuscitation (CPR) certification.
- Knowledgeable of community resources and previous experience in a social service agency or Community Health Nursing desirable;
- Valid driver’s license and access to an automobile during working hours that meet the State of New Hampshire registration regulations and auto liability insurance
