What are the responsibilities and job description for the Care Plan Manager / Coordinator position at Home & Hearth Caregivers?
Home & Hearth Caregivers is a non-skilled in-home care agency licensed by the Illinois Department of Public Health. We provide in-home care to our clients so they can age in place with dignity. As our company grows, we are seeking a Care Plan Manager to take inbound calls from potential prospects and transition them into clients that we provide care for. The day to day activities include following up on previous prospects and providing in-home assessments to assist them in deciding what type of care is best for them. Ongoing support and follow up with existing clients is required to make sure we are providing the best care possible. This can be stressful at times, but the rewarding aspects of this position far outweighs any stress. Even though this position is healthcare related, there are no special healthcare related skills this position requires. A great personality and the ability to relate with all types of personalities is the key to being successful in this role.
In addition to the benefits listed, a company vehicle and gas allowance will be provided after 90 days of employment.'
